Are you having trouble with your online subscription? Here are some frequently asked questions that may help to answer your question.

What if I click the "Submit Form and proceed to payment options" button and I receive an error message?  What if NOTHING happens and I remain on the same page?

  1. A correctly submitted form will be followed by a confirmation page.  If you do not receive this page, see below.
  2. There may be a required field that needs to first be filled out prior to submitting the form.  Check to ensure all fields have been completed and try submitting the form again.
  3. If this does not solve the problem, close the web page and then return to the form in a new window.  Fill out the form and submit it again.
  4. If this does not solve the problem, contact and explain your situation.

I accidentally submitted the first part of the online form without completing the information.  What should I do?

Don't panic.  Go back to the form and complete it accurately.  In the "Additional Comments" field at the bottom of the first form, let us know that an incomplete form was sent and we'll replace it with the corrected one.

I submitted the first form but had trouble with the credit card portion.

  1. This could result from a variety of errors.  Be sure that the name on the account and the billing address you enter matches the official billing name and address of the card.  Ensure that the card number and expiration date you entered are correct.  If this does not solve the problem, see below.
  2. Perhaps your credit card issuer has suspended your account.  Contact your card issuing company to determine if there is a problem with your account.
  3. If this does not solve the problem, contact and explain your situation.
